How to make a referral
All referrals to the LINK Transition Center must be completed by a student's school district. The school district administrator responsible for Special Education services should submit a LINK Student Referral & Intake Form, along with the required documentation.
✅ Current IEP with Transition Plan
✅ Current Triennial Evaluation
✅ High School & Beyond Plan
✅ Behavior Intervention Plan and/or Health Care Plan (if applicable)
✅ LINK Transition Center Consent Form
Once the intake form and documents are received, the Transition Services Program Manager will confirm submission and schedule a meeting to discuss student enrollment.
